Hope provides care by allowing maximum freedom and independence in your place of residence. We also personalize each individual’s hospice plan under the direction of the physician. Our team of nurses and professionals evaluates and set goals tailored to meet your needs. This will provide comfort care and peace of mind for your loved one.

What is
Hospice?
Is now the time for hospice or palliative services? Each person has needs, which is why Hope Hospice and Palliative Care work with families to help make decisions and determine the best level of care. Our accredited palliative and hospice care program delivers services to the patient in their home, nursing home, or other location. Ultimately, our goals are to provide comfort and celebrate the lives of our patients.

Includes bathing, dressing, feeding, bathroom and hygiene assistance, incontinence care, and bed transferring position.
Pharmaceutical options include oral medication and transdermal patches.
Supplementary alternative therapies as needed, including touch, massage, music, and aromatherapy.
Registered Nurses (RNs) provide professional nursing and medical care under the supervision of an attending physician.
In times of grief, professionally trained grief counselors provide consolation and coping tools to family members and friends.
Social Workers are available to assist with community resources as well as Spiritual Counselors to help with the grieving process.
When Can I join Hospice Care?
You may enter a program if your doctor states that you have a terminal illness and that death can be expected in 6 months or less. You can stay in a hospice beyond that time If your doctor and the team determine you still qualify for services.
